Substance Misuse Co-occurring Conditions Programme Manager

Role Responsibility

This is a full time 18 month fixed term (funding dependent) post for a Project Manager for Substance Misuse / Co Occurring Conditions in Kent.

This is an exciting opportunity for a person committed to improving the lives of people with complex vulnerabilities including addictions, mental illness, housing, social and emotional trauma and physical health problems. Working in a client focused way, you will help the client and the health system work together to create better outcomes for vulnerable people. You will co-ordinate multi agency leads to create shared care plans for vulnerable people identified by both substance misuse and mental health providers. You will be familiar with the NHS co-occurring conditions quality standards and hold all partners to account as part of ongoing public health quality improvement to enable people to access the right care/treatment at the right time. You will work in a client focused way alongside local commissioners, public health services, mental health and substance misuse workers to ensure the care for service users is safe, appropriate and co-ordinated.

You will advocate for the client and ensure that the service user has a shared care plan in place and that any barriers to care are highlighted and documented and where appropriate - removed in order for the service user to get the care they need. You will be familiar with and able to work across a range of agencies, as well as demonstrating commitment to the needs of this client group. You will be supported by the public health team and have good access to leaders in the mental health system. You will also contribute to Kent and Medway's award-winning suicide prevention programme.
