Arranging Support Coordinator

Join our Arranging Support Hospital Discharge Team as a Coordinator and play a pivotal role in helping people move from hospital to the right care home.
Working in a fast-paced and highly pressured environment, you will manage and prioritise hospital discharge referrals, working swiftly and efficiently to arrange residential or nursing care placements that suit each individual’s assessed needs. Working closely with local health and social care teams, providers, families, and individuals, you’ll coordinate seamless transitions from hospital, ensuring people and their representatives understand their choices and the costs involved. Your role will also include supporting and managing contract and placement information.
Key Responsibilities:
- Manage referrals following KCC procedures
- Liaise with Locality Teams, health colleagues, providers, and families
- Provide clear information on accommodation options and costs, undertaking light financial assessments as needed
- Act as the main contact with providers and ensure secure data sharing in line with GDPR
- Monitoring bed usage and placement effectiveness

- Confidence in managing multiple stakeholders and sensitive information

- Ability to work collaboratively and flexibly within a multi-disciplinary team
