Social Worker - NHS Continuing Health Care Liaison

Role Responsibility

An exciting opportunity has arisen to support the development of NHS Continuing Health Care (CHC) and joint funded care and support planning within the Adult Social Care and Health (ASCH) workforce.

We are looking for a dynamic and confident practitioner who is keen to develop their career within KCC and has a specific interest in NHS Continuing Healthcare. The successful candidate will require a detailed understanding of the NHS CHC Framework, complex case work and experience of supporting the practice of registered staff.

You will promote consideration of NHS CHC eligibility and joint funding at the right time through developing practice and completing targeted reviews with people currently in receipt of social care services across Adult Social Care and Health. You will actively drive the identification of people already in receipt of local authority funding who are likely to be eligible for NHS CHC or joint funding and promote creative solutions to meet individual care and support needs.

The role requires excellent communication, relationship building, influencing and negotiations skills as you will be working in partnership with professionals both internal and external to the Council.

A detailed working knowledge of the National Framework for Continuing Health Care is essential.

The successful candidate will be able to demonstrate an ability to manage their own time, delegate and prioritise their own workload and be registratered as a social worker with Social Work England.

Post holders are responsible for ensuring that all Council Adult Safeguarding Policies are adhered to and that concerns are raised in accordance with these policies.

You must also demonstrate commitment to support the Directorate’s Equality and Diversity Policy Statement which is an integral part of the Directorate’s service delivery.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.
