Social Worker Assistant

Role Responsibility

The Homes not Hospitals Team work to plan the discharges of individuals with a Learning Disability and/or Autism who are part of the Homes Not Hospitals cohort. There is a national drive to find the right homes and the right support for people within the community and reduce hospital admissions and length of stay. 

Kent County Council is looking for a social work assistant to be part of this innovative team, improving the lives of people with a learning disability and/or autism who are in specialist hospitals.

This is a rare chance to be part of this service, providing opportunities to develop your skills and your career. You will play a key role in the multi-disciplinary team assisting social workers to deliver the Local Authority’s statutory duties to support the transition of individuals with learning disabilities and/or autism out of hospitals and into the community.

You will need to be able to communicate effectively both with the clients, their families and other professionals. Your role will include tasks such as, enabling individuals to achieve their goals in life, supporting them to maximise their benefits and find opportunities to help them integrate into their community, undertaking reviews as per the Care Act 2014, supporting the individuals to find a suitable home for them in the community and attending some meetings such as ward rounds, Care Programme Approach meetings and Care and Treatment Reviews on behalf of the allocated social workers. You will be proactive and independent in your work, but be confident to seek support, advice and to joint work with others when required.

You will have experience in either mental health, learning disability or autism settings. Applications will be shortlisted in relation to Job Description/Person Specification, which should be addressed fully in your application.

This is a permanent, full-time role. This is a countywide service, and your base will be the closest KCC office to your home. You will be working flexibly, including from home and across a wide geographical area so the ability to travel in a timely manner according to business need is essential.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
