Case Officer – Short Term Pathways Team – East Kent Area

The Short Term Pathways Team works very closely with partner agencies to improve patient transition pathways. As a Case Officer in the team you will work alongside the Rapid Transfer of Care Team and manage a small caseload with a fast turnover of adults who are currently in an acute or community hospital, or other short-term care setting.
The Rapid Transfer of Care team operates a discharge to assess model of care which involves patients having their assessments outside of the acute hospital environment and you will be supporting clients as they move through and beyond this pathway. You will work within a multi-professional, supportive environment where knowledge and skills are shared to achieve the best possible outcomes for individuals.

The new Adult Social Care and Health service operates with modernised office processes and performance management systems. It is increasingly accessible to the public outside of traditional office hours, as we move towards an 8am to 8pm, 7 days a week service to ensure service needs are met.
This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

Our offer
- Starting salary £22,581 to £25,564 depending on experience
