Case Officer – Short Term Pathways

This post would be based in the Swale area and focusing on the work for discharge planning for clients under the Older Persons Physical Disability (OPPD) remit . This post is within a team that works collaboratively with health and other partnership agencies and focuses on promoting a persons independence.
We are looking for motivated, proactive professionals who are interested in working within the Short Term Pathways, and keen to work with Acute, Community Hospitals. The successful applicant also needs to be flexible in their approach and committed to working in an integrated team and ready to embrace new ways of working, to help further shape our services.
As a Case Officer working with people over the age of 18 with a wide range of needs, you will manage a caseload requiring assessment, review and on-going support to make a real difference in achieving improved outcomes, promoting the independence of our service users.

Driving is a core function for this position.
This post is considered by KCC to be a customer-facing position. The Council
therefore has a statutory duty under Part 7 of the Immigration Act (2016) to
ensure that post holders have a command of spoken English/Welsh sufficient for
the effective performance of the job requirements. The appropriate standards are
set out in the Job Description/Person Specification.
We are increasingly accessible to the public outside of traditional office hours, as we
move towards an 8 am to 8 pm, 7 days a week service to ensure service needs
are met.
This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service.
This post is in Swale and there is an expectation that the successful applicant will travel to the various locations in Swale and Kent to undertake short term pathway work.
