Social Worker OPPD – Short Term Pathways Team

The post is based in the East Kent area, focusing on the work for discharge planning for clients under the Older Persons Physical Disability (OPPD) remit. This post is within a team that works collaboratively with health and other partnership agencies and focuses on promoting a person’s independence.
We are looking for a motivated, experienced professional who is interested in working within the Short-Term Pathways Team, and keen to work with Acute and Community Hospitals. The successful applicant also needs to be flexible in their approach and committed to working in an integrated team and ready to embrace new ways of working, to help further shape our services.
As a Social Worker within the Short-Term Pathways Team, you will be working with people over the age of 18 with a wide range of needs, you will manage a caseload requiring assessment, review, and on-going support to make a real difference in achieving improved outcomes and promoting independence.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. You should note that public transport is rarely a viable option as some rural areas of Kent often have limited public transport.
This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
We are increasingly accessible to the public outside of traditional office hours, as we
move towards an 8am to 8pm, 7 days a week service to ensure service needs
For internal applicants only, an Occupational Therapy or Nursing qualification will also considered eligible.
This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.
This post is linked to the Acute and Community Hospitals in East Kent. One post is linked to the Kent and Canterbury Hospital, the other is linked to our Community sites - although work base will be discussed further at interview. There is an expectation that the successful applicant will travel to various locations in East Kent to undertake Short-Term Pathways work.
- Starting salary £30,034 - £40,405 per annum
- Full induction and comprehensive professional learning and development
- Regular and supportive supervision
- Starting annual leave allowance of 27 – 30 days
- Consideration for one-off, market recruitment payment to successful applicants who meet the criteria.
