We are looking for an experienced professional person to join our Short-Term Pathways Team – so if you are committed to working in integrated teams, ready to embrace change and new ways of working this could be the opportunity for you.

The Short Term Pathways Team works very closely with partner agencies to improve patient transition pathways. You will work within a multi-professional, supportive environment where knowledge and skills are shared to achieve the best possible outcomes for individuals.

We are looking for a motivated professional who is interested in building strong relationships with our partners. You need to be flexible and committed to working in an integrated team and ready to embrace new ways of working to help further shape our services.

If you are a registered Social Worker (or meet the qualification criteria as a KCC employee) and would like to be a part of the Short-Term Pathways Team during a time of exciting and innovative change then we would love to hear from you.

The post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and a relevant professional qualification as well as up to date registration with the appropriate professional body.
