Social Worker - Short Term Pathways

We are looking for an experienced professional person to join our Short-Term Pathways Team – so if you are committed to working in integrated teams, ready to embrace change and new ways of working this could be the opportunity for you.

The Short Term Pathways Team works very closely with partner agencies to improve patient transition pathways. As a Social Worker in the team, you will manage a small caseload with a fast turnover of adults who are currently in a short-term care setting. You will have the opportunity to undertake holistic assessments and be creative with care and support planning in order to support people to be discharged from these care settings on to the best pathway for their recovery and to maximise their independence. You will work within a multi-professional, supportive environment where knowledge and skills are shared to achieve the best possible outcomes for individuals.

We are looking for a motivated professional who is interested in building strong relationships with our partners. You need to be flexible and committed to working in an integrated team and ready to embrace new ways of working to help further shape our services.

If you are a registered Social Worker (or meet the qualification criteria as a KCC employee) and would like to be a part of the Short-Term Pathways Team during a time of exciting and innovative change then we would love to hear from you.

The post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and a relevant professional qualification as well as up to date registration with the appropriate professional body.

Whilst mainly working from home at the current time, a fixed working base will be discussed at interview.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential.

Applicants are required to have received both Covid vaccinations.

We are increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met whilst operating under a flexible working policy.

Our offer

  • Starting salary £30,034 to £40,405 depending on experience
  • Full induction and comprehensive professional learning and development
  • Regular and supportive supervision
  • Starting annual leave allowance of 27 to 30 days
