Social Worker OPMH – Short Term Pathways

The post would be based in the North Kent and Swale area and focusing on the work for discharge planning for clients under the Older Persons Mental Health (OPMH) remit. This post is within a team that works collaboratively with health and other partnership agencies and focuses on promoting a person’s independence.

We are looking for motivated, experienced professionals who are interested in working within the Short Term Pathways, and keen to work with Acute, Community Hospitals. You also need to be flexible in your approach and committed to working in an integrated team and ready to embrace new ways of working, to help further shape our services.

As a Social Worker within the Short Term Pathways team you will be working with people over the age of 18 with a wide range of needs, you will manage a caseload requiring assessment, review and on-going support to make a real difference in achieving improved outcomes and leading to independent living for the service user.

Our team in Short Term Pathways is creative, dynamic and diverse. The team are proud of the high standard of service we provide to our customers at their most vulnerable times.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. You should note that public transport is rarely a viable option as some rural areas of Kent often have limited public transport.

We are increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.

This post is based at Darent Valley Hospital, however there is an expectation that you will travel to the various locations in North Kent and Swale to undertake short term pathway work.
