Social Care Discharge Coordinator - West Kent

As a Social Care Discharge Coordinator you will work in partnership with the Integrated Discharge Team to support to manage the discharge pathway for a population of clients moving through the acute hospital setting, working closely alongside other agencies and professionals. 

You will be responsible for triaging and allocating referrals to appropriate workers in order to facilitate safe and timely discharges for people who are on a hospital discharge pathway. You will supervise and provide complex case management advice to a team of Registered Practitioner Case Officers. You will also be responsible for accurate data collection and escalating any issues affecting functionality of the team to the Senior Practitioner and Team Manager. 

The Integrated Discharge Team operates a discharge to assess model of care which involves patients having their assessments outside of the acute hospital environment and you will be supporting clients as they move through and beyond this pathway. 

Adult Social Care and Health operates within modernised office processes and performance management systems. The service is increasingly accessible to the public outside of traditional office hours, as we move towards an 8am to 8pm, 7 days a week service. OPPD staff and managers are increasingly working on a rota basis to ensure service needs are met.

This role is subject to holding a full UK Driving Licence.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.
