Social Care Discharge Coordinator - Short Term Pathways

Please Note: The application deadline for this job has now passed.

Role Responsibility

We are looking for a motivated, experienced professional to join us in our work in partnership with multi-agency teams. If you are a Social Worker (or an existing KCC employee who meets the qualifying criteria) with experience in Adult Social Care, and interested in embarking on a new career opportunity, ready to embrace change and new ways of working then we would love to hear from you.

As a Social Care Discharge Co-ordinator, you will work alongside NHS colleagues to manage the discharge pathways for individuals moving through the community hospital setting as well as with individuals who are in a short-term placement in one of KCC’s care homes. This post involves working closely with other agencies and professionals in order to achieve this.

You will be responsible for triaging and allocating referrals to appropriate workers. You will supervise and provide complex case management advice to a team of Case Officers and Registered Practitioners. You will support the team to deliver a high quality of practice and innovative person-centred outcomes for the residents of East Kent. You will also be responsible for accurate data collection and escalating any issues affecting functionality of the team to the Team Manager.

The service is increasingly accessible to the public outside of traditional office hours, as we move towards 7 days a week service. OPPD staff and managers are increasingly working on a rota basis to ensure service needs are met. This is an exciting time to join us and informal discussions prior to interview are welcomed from people who share our values and are ready to be part of our exciting journey.

  • Full induction
  • Starting annual leave allowance of 27 to 30 days
  • A minimum of 5 days’ training each year

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

Please note that due to the current situation, the interview may not be held face to face but via telephone/webcam.
