Senior Practitioner - Adult social care connect

We operate in a hybrid model of office-based working and working from home to meet the needs of the service.

About Us

An exciting opportunity has arisen for a Senior Practitioner to join the Adult Social Care Connect Team covering the Thanet & South Kent Coast areas.

The Team provides initial contact following a referral from a range of sources and works with adults with a range of care and support needs.  Our team focuses on supporting people when they first contact Adult Social Care Services to ensure proportionate assessments are undertaken where indicated and arranging urgent support to manage eligible social care needs as required. 

Our social care interventions are structured around the three pillars of excellent practice, exceptional innovation, and meaningful measures. Therefore, our work is orientated around building relationships with the individual, their families and carers. We engage with communities early on to help people feel empowered to find trusted help and support locally from a range of sources.

The successful applicant would support with initial triage of referrals and provide supervision, direction and guidance to both registered and non-registered staff as they undertake assessments.  You will bring professional expertise, skills and advice to enable the provision of an effective, high quality social care service that meets KCC and multi-agency policies, procedures and practices.

The role will involve providing professional supervision and guidance to both Registered Practitioners and Social Care Officers to promote best practice in line with our statutory duties.

The office base for this post is Cheriton House, Folkestone with a hybrid working model in place.

About You

We welcome applications from experienced social workers who can demonstrate post-qualification experience in Social Services, Health related agencies or related private or voluntary organisation.

You will hold a registration with a professional body (Social Work England, HCPC,NWC) and be competent to work at the Advanced Practitioner level of the Professional Capabilities Framework for Registered Practitioners

You will have experience of managing risk and completing safeguarding enquiries

You will be able to demonstrate expertise in the supervision and development of staff, including performance management

We are increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and a relevant professional qualification (Social Work, Nursing or Occupational Therapy) as well as up to date registration with the appropriate professional body
