Senior Practitioner - Short Term Pathways - West Kent

The post would be based in the West Kent area and the focus of the Short Term pathway team is to support people who are on a hospital discharge pathway.

We are looking for motivated, experienced professionals who are interested in working within the Short-Term Pathways, and keen to work with Acute, Community Hospitals. The successful applicant also needs to be flexible in their approach and committed to working in an integrated team and ready to embrace new ways of working, to help further shape our services. 

As a senior practitioner within the Short-Term Pathways team, you will be providing

  • Practice and supervisory service to a designated group of professional staff in the Short-Term Pathways team, ensuring effective safe practice, high quality case work / service and performance in the designated team through the application of professional expertise to the team and wider teams. Ensure adherence to the operating framework for OP/PD clients and related policies, procedures, and practice guidance.
  • Management of a client caseload made up of highly complex / safeguarding cases that require a high level of specialist expertise to ensure that a proportionate assessment is undertaken to identify required needs, risks, and outcomes, to promote enablement and ensure an appropriate level of resource allocation is made to meet people’s support needs.
  • The ability to work closely with other social and health colleagues, including the teams Occupational Therapists to promote independence throughout recovery.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

We are increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.

We also welcome C.V applications from experienced registered nurses and occupational therapists with relevant experience into our Practitioner roles.
