Role Responsibility

The South Kent Coast Adult Social Care service is currently focussed on supporting individuals with social care needs arising from mental health difficulties or Autism and is looking to appoint Registered Practitioners.

Our social care provision in Adult Services is evolving and introducing exciting developments that are enabling us to provide a service based on national guidance, placing the person at the centre of our practice. Through the application of strengths-based practice we seek to collaborate with our colleagues across all avenues of adult social care provision to make a difference every day in people lives by providing interventions that wrap around them, are local in delivery and aim for them to only tell their story once.

You will be co-located with health colleagues and work alongside these as well as other multi-disciplinary colleagues and agencies, including Acute and Community Trusts, Primary Care Services, and the voluntary sector.  Your base will be either Ash Eton, Folkestone, or Coleman House, Dover.  

Your role will involve offering assessments, and interventions under the Care Act 2014 to the individuals we work with who have unmet social care needs or need advice and guidance to prevent needs arising.

Your role will also include supporting service users and carers in co-production to develop personalised care and support plans, assessing, planning and implementing these. You will also evaluate appropriate recovery plans which draw on community resources to achieve the most positive outcomes, as well as providing strength-based interventions, problem solving, connecting families and communities for the individual’s benefit.

This role will also involve undertaking some short-term assessments and interventions, advice and guidance, focussing on a strength-based approach which maximises self-determination and empowerment.

We value our staff and provide regular supervision, reflective practice and support to progress within the role offering plenty of opportunities to both undertake and contribute to training initiatives and in-house core training courses.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and a relevant professional qualification (Social Work, Nursing or Occupational Therapy) as well as up to date registration with the appropriate professional body.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
