Role Responsibility

An exciting opportunity has arisen within Kent County Council’s Strategic Integrated Commissioning Team for a Project Officer to support the implementation of system wide Dynamic Support Registers (DSR) across Kent & Medway for 6 months.

The NHSE Service Model for Commissioners of Health and Social Care Services (2015) stipulates that local health and care services should develop a dynamic register based on sophisticated risk stratification of their local populations. This will enable local services to anticipate and meet the needs of those people with a learning disability and/or autism who display behaviour that challenges, or who are at risk of developing behaviour that challenges, ensuring local services plan appropriately and provide early interventions, including preventative support.

As a Project Officer you will support the work at pace with the Senior Commissioner alongside multiple stakeholders across Health, Social Care, Education and the Criminal Justice system to ensure Kent & Medway achieves its Integrated Care System (ICS) objectives in delivering functionally of 2 x DSRs (Adults and Children & Young People) to support its Learning Disability and or Autism (LDA) programme outcomes of admission avoidance and discharge from out of area/inpatient settings.

You will have experience in delivering bespoke projects, be accustomed to working across a range of sectors and multiple stakeholders within change management programmes and have a successful track record of helping to deliver successful projects and outcomes.

Knowledge and or awareness of neurodevelopmental conditions would be an advantage, as would experience of working with or in health, social care or Educational settings.

(Please note due to Covid legislation the post holder will be required to work from home or a local Covid-secure building for the majority of their working day and/or attend face-to-face meetings as requested, subject to Covid-secure arrangements being in place. The majority of networking and formal meetings under Covid arrangements will be conducted through online Microsoft Teams Calls)
