Practice Consultant: Transition 18 - 25yrs

Are you a motivated Health, Education or Social Care Professional with a good range of experience in the arena of supporting Young People with complex neurodiverse conditions and are you looking for a new challenge and wanting to develop/enhance your experience working at strategic and system level?

NHS Kent and Medway in partnership with Kent and Medway County Councils have already invested and appointed five Practice Consultants, to support the mobilisation and development of their Designated Key Worker Programme. The current vacant Practice Consultant role is an opportunity to work systemically with young people,18-25yrs, and their families. Supporting community health, education, and social care teams to improve co-ordination of their service response through timely discharge planning, interventions, and risk assessment. Reducing risks of admission to specialist hospitals or forensic intervention and preventing further escalation of risk for those young people displaying behaviour that challenges.

This is a system support role and would best suit practitioners who are currently an experienced professional in their field of practice looking to extend their skill set working with 18-25 years and would enjoy a new challenge working within the transition through early adulthood arena.

This role will be a 2-year fixed term contract with expressions for secondment welcomed.

Secondment applicants will need to have the consent of their employing organisation.
