Health and Care Integration Lead

Role Responsibility

The System Discharge Pathways Programme has been in place for a year with a wide stakeholder group involved in delivering effective discharge pathways from acute and community hospitals, and delivering against a set of recommendations for the delivery of consistent, needs-based services aligned to the Integrated Care System.

Kent County Council and NHS Kent and Medway are seeking an experienced senior manager who will be will be instrumental in shaping and driving forward medium to longer term improvements as well as shaping the future of health and care integration for Kent.  

We are seeking an exceptional individual who has the energy, ideas and ambition to make a sustained difference to the people of Kent; people who share our commitment to improving the health and wellbeing for Kent residents. 

Working across the four Health and Care Partnerships in Kent and Medway, the post holder will build effective relationships with four acute NHS Trusts, three community health providers, adult social services as well as engagement with the voluntary and community sector to support the implementation of a range of new models of care, standardised discharge pathways and ways of working to support delivery of the System Discharge Pathways Programme. You will be able to influence a range of stakeholders to deliver effective services to the people of Kent.

You will hold significant experience in leading system level transformation and delivering change programmes and have experience working within a health and/or social care sector at a senior level with a good understanding of the context and aims of current health and social care economy across Kent.

Interviews will be held in two stages and the first stage is scheduled for week commencing 31st October 2022.  If successful, the second stage interviews will be held week commencing 7th November 2022. As part of the interview process for this role you may be required to complete additional testing.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
