Family Hub Practitioner (11-19)

Role Responsibility

At Kent County Council, our Family Hubs are vibrant community spaces where families with children aged 0–19 (or up to 25 with SEND) can access a wide range of support. From midwifery and health visiting to parenting advice and youth activities, we’re here to help families thrive. With 56 hubs across Kent, we work hand-in-hand with partners to make a real difference.

Family Hub Practitioner 11–19 (up to 25 with SEND)
Are you passionate about making a real impact in the lives of young people and their families? Do you thrive in a fast-paced, community-focused environment? If so, we want you to join our team as a Family Hub Practitioner. We are based in Swanley Family Hub but travel will be required to our other Family Hub in Edenbridge.

About the Role:
As a Family Hub Practitioner, you’ll work within our Family Hub team, providing 1:1 and group support to young people (aged 11–19, or up to 25 with SEND) and their families across Swanley and Edenbridge. You’ll collaborate closely with Integrated Children’s Services and a range of multi-agency partners—including schools, police, and community organisations—to improve outcomes for vulnerable young people.

You’ll deliver focused support through both in-person and digital channels, helping families navigate challenges such as mental health, exploitation, substance misuse, and family relationships. You’ll also lead parenting workshops, facilitate peer groups, and contribute to safeguarding strategies within the district.

Key Responsibilities:

  • Plan and deliver targeted interventions for young people and families, including digital and online support.
  • Work collaboratively with multi-agency partners to address safeguarding and contextual risks.
  • Support families in accessing digital services and community resources.
  • Lead and deliver parenting programmes and workshops.
  • Maintain high standards of record keeping and contribute to assessments and plans.
  • Ensure the voice of young people is central to all support and service design.

About You:
You’ll have a Level 3 Diploma (or equivalent) in Youth Work, Health and Social Care, or a related field, or substantial relevant experience. You’ll bring experience working with vulnerable young people and families, excellent communication and organisational skills, and a commitment to safeguarding and inclusion.

Working Arrangements:
This role requires flexibility, including occasional evening and weekend work, and travel between Swanley and Edenbridge Family Hubs.
