Face-to-face Pre-Admissions Assessor
Role Responsibility
An exciting opportunity has arisen to join the team at Broadmeadow Registered Care Centre as an Admissions Assessor
Broadmeadow is a 48-bed adult short stay setting, regulated by the Care Quality Commission and owned and run by Kent County Council. We provide respite, rehabilitation and assessment to adults with a physical need, and/or, cognitive impairment.
The post-holder will provide a timely response to all incoming referrals, screening against eligibility criteria, and our current dependency levels. They will work to achieve a high level of occupancy, providing supporting risk assessments and initial care plan assessments to ensure we can safely meet the needs of the individual being referred.
The role includes regular liaison with acute hospital settings, KCC's placements team and a range of social care practitioners. You will be able to commute within the Kent area to complete face-to-face assessments where necessary, attend regular multi-disciplinary meetings to strive for enablement, independence and high quality care for those we support, facilitating a safe and timely discharge.
You will provide regular updates to the management team regarding dependency levels, admission and discharge plans and support a range of audits and other ad-hoc management support. Therefore excellent computer skills and a fluency in English are essential for the role.
Candidates will hold a minimum of Level 3 NVQ in Health and Social Care and willingness to work towards a Level 5. Experience of staff supervisions, awareness of the Care Act, Safeguarding and CQC regulations are desirable.
Kent County Council offer an attractive employee package with access to pension scheme, generous annual leave, full induction and training and other staff benefits. Hours to be worked are typically between 08:30 - 17:00 from Monday to Friday - to be agreed at interview.
This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.
