Face to Face Assessor
Role Responsibility
An exciting opportunity has arisen for a Face to Face Assessor to complete assessments of prospective service users to access Westbrook House Integrated Care Centre in Margate. Westbrook House offers Enablement and Dementia services for respite and assessment, currently.
You will be expected to:-
- Complete assessments of prospective service users for the short stay units or day care service and inform the Registered Manager of your decision to admit or decline with clear evidence as to your reasons.
- Manage and coordinate referrals and assessments and carry out face to face assessments.
- Collate information and input onto computer systems.
You will have to prepare the initial risk assessments and care plans before the service user is admitted to the service. You will be a key player in MDT meetings, ensuring each service user is reviewed weekly and as part of their moving on plan leaves the service at the right time for them having achieved their goals. You will provide higher level management support to the Registered Manager and Senior Team Leader so that the centre delivers high quality, safe care and maintains a customer friendly service; this will include taking part in quality assurance and audits and at times supporting the senior rota.
There is a need for you to have a minimum qualification of level 3 diploma in health and social care or similar with a willingness to work towards your level 5 diploma. You will also need substantial experience of working with the older person with supervisory experience in a social care setting, together with good communication and interpersonal skills including report writing.
Knowledge of all the relevant legislation affecting individuals we support is essential as well as an understanding of national standards, regulations and requirements of external regulators, relevant policies/procedures in relation to Safeguarding, Duty of Candour, Health & Safety, Mental Capacity Act, Deprivation of Liberty safeguarding and Data Protection (GDPR). A good knowledge and understanding of the needs, management and planning for older people including relative policy procedure and legislation is also required.
Interviews will follow the Choosing with Care process. Invited candidates will be required to attend a full day which will include the formal interview as well as written and numerical testing, profiling and discussion group.
