Experienced Social Worker
Role Responsibility
The Homes not Hospitals Team works to discharge plan from hospital for people with a Learning Disability and/or Autism who have been identified as part of the Homes Not Hospitals cohort.
You will play a key role in delivering the local authority’s statutory duties, including assessing eligible social care and section 117 needs and acting as social supervisor where necessary (training will be provided). Utilising strengths and rights-based approaches you will support people to reintegrate into their communities, reduce inequalities, improve quality of life, instill hope and realise real change.
You will have experience of working with complexity, diversity and of assessing and mitigating risk and of working alongside health and third sector partners to develop robust aftercare plans. You will be an Approved Mental Health Professional and/or a Best Interest Assessor and/or a Social Supervisor.
You will manage a caseload, acting as social supervisor when required (training will be given), and will regularly review and evaluate social care interventions and services to ensure needs continue to be met in the most appropriate way.
The Homes not Hospitals Team is based at the Trevor Gibbens Unit in Maidstone, Kent but you will be working flexibly across a wide geographical area including from home, so the ability to travel in a timely manner according to business need is essential.
