Experienced Social Worker KSH (Partnership Working with Gravesend Rough sleeping team)
We are recruiting an Experienced Social Worker KSH for the Gravesend Community Team, in partnership with Gravesend Borough Council. This position offers a 12-month Fixed term Contract.

The Rough Sleeping Strategy for Gravesham Borough Council provides a comprehensive plan to address rough sleeping in alignment with the Department of Levelling Up, Housing, and Communities (DLUHC) guidelines. It focuses on delivering services to support individuals at risk of homelessness, ultimately aiming to end rough sleeping in the borough.
The Social Worker will be based at Gravesend Borough Council office (Civic Centre) with the Rough Sleeping Team (RSI).
The Rough Sleeping Team :
- Identify and support individuals that are homeless or at risk of rough sleeping who present with care and support needs ensuring accessibility to all members of the community regardless of the immigration status.
- Holistically engage with people in a variety of settings, including ‘on the street’, in community settings such as day centres, HEAL House and in more structured settings such as the Civic Centre.
- Creatively find ways to engage with clients whose care and support needs are a barrier to them accessing services.
- Complete Care Needs and Capacity Assessments, along with Addenbrooke’s Cognitive Examination III (ACE‐III) for the detection of dementia and mild cognitive impairment where necessary.
- Work with mental health services and Drug and alcohol services to create a multi-agency approach.
- Complete support plans and additional referrals where necessary.
- Both KCC and GBC will be working within the Multi-Agency Safeguarding Adults Policy. Both agencies will co-operate to ensure the safety of vulnerable adults though this framework guided by their own and each other’s policies and procedures. This includes safeguarding and self-neglect.
- Provide support to those under the RSI Service who have been identified as having care and support needs, whether they are actively rough sleeping, in supported/temporary accommodation or are housed but their mental health or care needs are attributing to them being at risk of homelessness.
- Attend Early Morning Outreach sessions where necessary, particularly to engage non-engagers who have been identified as having care and support needs.
- Record and report all communications with individuals and send to the allocated worker.
- Participate in the Bi-monthly and yearly outreach support.
- Record and provide quarterly monitoring returns to GBC.
Supervision and equipment will be provided by Kent County Council.
Candidates must meet the criteria outlined in the Job Description Specification.
You will have a high level of literacy and communication skills, both verbal and written.
This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.
This post is considered to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

£3000 Golden Hello payment to all new joiners (subject to criteria) and service related payments following continuous service.
