Experienced Social Worker- Homes Not Hospital Service

Role Responsibility

The Homes not Hospitals Team works to discharge plan from hospital for people with a Learning Disability and/or Autism who have been identified as part of the Homes Not Hospitals cohort.

We are looking for a KR10 Experienced Social Worker to fill a full-time post from 1 April 2025.

You will play a key role in delivering the local authority’s statutory duties, including assessing eligible social care needs, undertaking Mental Capacity assessments, completing community DoLS applications, assessing Section 117 needs and acting as social supervisor where necessary. Utilising strengths and rights-based approaches, you will support people to reintegrate into their communities, reduce inequalities, improve quality of life, instil hope and realise real change.

You will have experience of working with complexity, diversity and of assessing and mitigating risk and of working alongside health and third sector partners to develop robust aftercare plans. We are particularly interested in attracting applications from people who have experience/expertise in working with people who have a learning disability or autism but we welcome applications from across social care. You will be an Approved Mental Health Professional and/or a Best Interest Assessor, or have completed the KR10 portfolio.

You will manage a caseload, acting as social supervisor for Conditionally Discharged persons when required (training will be given), and will regularly review and evaluate social care interventions and services to ensure needs continue to be met in the most appropriate way. You will also act as supervisor for Registered Practitioners in the team. 

The Homes not Hospitals Team is based at the Trevor Gibbens Unit in Maidstone, Kent but you will be working flexibly across a wide geographical area including from home, so the ability to travel in a timely manner according to business need is essential.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.
