Enablement Assessment Officer

We have an exciting opportunity for a permanent Enablement Assessment Officer to join the Kent Enablement Services (KES) Home from Hospital team. This role offers a unique chance to work with people at a critical point in their mental health recovery journey.

About the Role

As an Enablement Assessment Officer, you’ll work with adults who are currently in psychiatric hospitals across Kent—including Littlebrook, Priority House, and St Martins—as well as Jasmine Ward in Dartford. You’ll begin enablement support while individuals are still in hospital and continue that support into the community after discharge.

Your role will be to empower people with mental health diagnoses, learning disabilities, and autism to regain and develop independent living skills in order to avoid future hospital admissions and achieve their personal goals.

What We’re Looking For

We’re seeking someone who is:

  • Empathetic and person-centred, with a passion for supporting people to live independently.
  • Experienced in working with individuals with mental health needs, autism, and learning disabilities.
  • Confident in working independently in hospital settings and collaboratively with multi-agency teams.
  • Organised, flexible and proactive, with strong assessment and planning skills.
  • Able to travel across Kent and adapt to changing needs and priorities.

What You’ll Need 

  • GCSE (or equivalent) A-C in Mathematics and English.
  • NVQ III or Diploma III in Health and Social Care or willingness to work towards (or relevant experience). 
  • Completed the Assessment Eligibility Criteria Care and Support Planning and Review training (or willingness to complete within 3 months) 
  • Trusted Assessor
  • Proven experience in a similar role.
  • A good understanding of the Care Act, Mental Capacity Act, and safeguarding procedures. 
  • Completed the Assessment Eligibility Criteria Care and Support Planning and Review training (or willingness to complete within 3 months)
  • Strong IT and communication skills.
  • A commitment to equality, diversity, and continuous professional development. 
  • Competent in literacy and numeracy
