Enablement Assessment Officer

We have an exciting opportunity for four full time Enablement Assessment Officers (KR7) two in East Kent and two in North Kent, as part of our expansion within Kent Enablement Services (KES) with an expectation to deliver the service Kent Wide if required.

Kent Enablement service is one of the leaders in its field and has the most comprehensive training programs as we believe in investing in both our staff as well as the residents of Kent.  

Enablement is all about taking an individual on a journey to independence or reducing their long-term care needs. This would be through planned support with aspects of daily living, whilst working alongside a structured team to include the Enablement Team Coordinators, Seniors, Support Workers, Business Support Teams, Social Workers, Occupational Therapist and various multi agency partners in KCC and health.  

The role requires you to hold an Assessment Eligibility Criteria Care and Support Planning qualification (or equivalent) aa well as experience of completing care needs assessments and support plans. You will manage a caseload of people with complex needs and  establish SMART Enablement goals  with them identifying the correct level of enablement to meet their needs and establish the outcomes required, to maintain or increase their independence.

 

This role is subject to holding a full UK Driving Licence.

 

The Enablement Assessment Officer will work closely with wider KES colleagues as well as our Occupational Therapist to ensure assessments are holistic and all opportunities are maximised.

 

This post is subject to an Enhanced Disclosure Application to the Disclosure and Barring Service.

 

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
