Role Responsibility

Want to take the next step in your career?

We are looking for individuals preferably with experience in the Health or Social Care sector to work as part of our team supporting adults in North Kent. If you are committed to working in integrated teams and ready to embrace change alongside new ways of working this would be the perfect challenge for you.

As part of a team you will work together with clients and carers to assess eligibility and if appropriate develop and implement care and support plans which will maximise independence. You will be making a real difference in achieving improved outcomes and promoting the independence of our service users.  You will ensure that clients and their carers views and wishes are central to the decision-making process.

The Older People Physical Disability (OPPD) service operates with modernised office processes and performance management systems. It is increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and two satisfactory references.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification

The role requires you to be mobile throughout a wide operational area including travelling around the county of Kent and beyond.  There may be times that you are required to travel across a wide geographical area, in a timely and flexible manner at various times of the day. You should note that public transport is rarely a viable option as some rural areas of Kent often have limited public transport.
