Admissions Assessor / Flow
Role Responsibility
An exciting opportunity has arisen for a full-time Admissions Assessor / Flow role (KR8) based within West Integrated Care Centre.
This is an interesting and exciting opportunity for the right candidate who will play an active role in all aspects of service delivery, alongside deputising for the Senior Team Leader / Manager when required.
You will need to be person centred, have a good understanding of the needs of adults, and adults with dementia and promote their rights and choices.
The role involves travelling in and round Kent, carrying out assessments of prospective service users, completing risk assessments before the service user is admitted, liaising with families, case managers, hospital coordinators, attending MDT meetings and any other duties relevant to the role.

The successful applicant will have an NVQ Level 3 in Health and Social care, or equivalent, and the Level 5 in Leadership and Management (or willingness to work towards) as well as appropriate relevant experience of working with adults and adults with dementia.
